Welcome to the third edition of the Residential Aged Care Communiqué for 2025 which includes two cases about medications and medication management. The first case is about a younger person in an aged care home on very long-term antipsychotic medication. The second case describes the risks with transitioning from paper to electronic medication charts. A common factor in both cases is our tendency to favour a familiar idea and become comfortable with a situation. The lesson is to know when to challenge the status quo.
