Welcome to the fourth and final edition of the Residential Aged Care Communiqué for 2025 which includes two cases about the inadvertent consequences of relaxing and tightening environmental restrictive practices. The causes of death in the two cases one from heat stroke and the other from a subdural haemorrhage. A longitudinal view of the contributing factors demonstrates the connections between the cases. The cases highlight the complexities of aged care and how the best of intentions do not guarantee the desired outcome.
